Resident Documentation and Recordkeeping
  • Maintain accurate and updated administrative records for each resident, including demographic data, appointment history, family contact logs, and care documentation uploads.
  • Ensure that resident care plans and compliance-related forms are tracked, organized, and accessible in digital systems.
  • Monitor and document resident status changes, admission/discharge activity, and room transitions based on facility protocols.
Family and Facility Communication Support
  • Coordinate non-clinical communications between facility staff, providers, and resident family members using secure messaging and shared systems.
  • Schedule and document regular updates or communications with family members as required by care protocols or compliance standards.
  • Track consent forms, emergency contacts, and escalation plans as part of resident recordkeeping responsibilities.
Appointment and Transport Coordination
  • Arrange medical or dental appointments in coordination with provider schedules, family preferences, and facility availability.
  • Coordinate transportation requests with third-party vendors, ensuring the correct timing, documentation, and follow-up are completed.
  • Track and document appointment outcomes or missed visits for future rescheduling and care plan updates.

  • Resident Administration Knowledge: Understands workflows and terminology associated with long-term care and assisted living environments (e.g., ADLs, care plans, occupancy logs).
  • Recordkeeping Proficiency: Accurately maintains census reports, admission/discharge logs, and compliance documentation in digital systems.
  • Family Communication Documentation: Tracks calls, messages, and status updates sent to and received from families to ensure traceability and professionalism.
  • Appointment and Transport Logistics: Schedules and tracks external appointments and transport coordination tasks using EHR, calendars, or shared facility tools.
  • Compliance Form Tracking: Ensures forms related to regulatory requirements (e.g., POAs, consent forms, care plans) are up to date and digitally stored.
  • HIPAA and Facility Privacy Compliance: Maintains privacy of all resident information in compliance with HIPAA and internal long-term care documentation standards.

  • Family-Facing Professionalism: Communicates with warmth and professionalism in interactions with families, supporting a respectful and informative experience.
  • Reliability and Follow-Through: Demonstrates accountability in maintaining schedules, records, and communication loops across all coordination tasks.

Client Work Requirements

Compensation Structure

Compensation for selected candidates typically ranges between USD $500 – $1,000 per month, depending on:

  • Total years of relevant experience
  • Role-specific competencies
  • Industry alignment

All compensation decisions are made at the discretion of the Talent Acquisition team, based on structured evaluation criteria and professional judgment. We are committed to maintaining a fair and non-discriminatory selection process.

Final compensation and placement decisions are determined at the time of client assignment and are considered final and non-contestable.
